A single item in the UNIFIED INBOX — every task, notification, and decision an actor needs to see, from any solution, in one place. The inbox is registry-backed; both case tasks and notifications write these.

`actions`	`array<string>`		allowed actions for this item (e.g. approve, reject, decide, contest). Each action produces a decided change-request → decision → receipt.
